How Is Data Collected
With the exception of cookies and logs, as detailed below, personal data that is collected about you is either collected on the medical form that you have completed, or in the form of notes taken by Michelle during classes or sessions that you attend with her.
What Data Is Collected
Your name, address, details of your next of kin, details of your doctor, and details of your medical history are collected. Further, details of what treatments or exercises you've taken part in, as well as which classes you've attended may be kept.
Why Is The Data Collected
The data is collected so that Michelle understands your medical situation so that she is best informed in order to be able to help you.
How Long Is The Data Kept?
The data is kept for as long as you are a client, and then for 5 years afterwards in case of any queries arising that may be related to any treatment or excersise you took part in. Once that period has expired, the paper files are destroyed either in a shredder, or on a bonfire.
How Is The Data Stored?
The data is all stored in paper files, locked in a filing cabinet in Michelle's house. Only Michelle has access to the key for the filing cabinet. Emergency contact details are kept on Michelle's phone, which is locked with a PIN.
Who Is The Data Shared With?
In short, no one. Only Michelle has access to personal information relating to classes or medical history. Karl runs the webserver and only has access to the webserver logs as detailed below. He does not have any access to any other data.
How Do You Make Requests About Your Data?
If you want to know what data is held about you, or you believe that some of the data is incorrect or inaccurate, please contact Michelle. For legal reasons, your data cannot be deleted/destroyed until after the 5 year period expires after you cease to be a client.
